The following information may be collected: Name, Email, Phone, Age.
Optional information for wellness support:
- General health goals
- Lifestyle habits
- Known allergies
- Medications currently being taken
- Herbal experience level
Providing accurate information helps practitioners offer appropriate wellness education.
Herbal Wellness Acknowledgment
Consultations provided by Moe's Butterfly Effect are intended to offer:
